HIPAA Privacy Notice

Dr. Claire Davis Plastic Surgery is dedicated to maintaining the privacy of your protected health information (PHI). We are required by law to maintain the confidentiality of your health information. This Notice of Privacy Practices describes how we may use and disclose your protected health information to carry out treatment, payment, or healthcare operations, and for other purposes permitted or required by law. It also describes your rights regarding your protected health information.

We may use and disclose your protected health information to provide, coordinate, or manage your healthcare and related services. This includes coordinating or managing your healthcare with other providers. For example, we may disclose your protected health information to another physician who may be treating you or to whom you have been referred to ensure they have the necessary information to diagnose or treat you.

You have the right to inspect and obtain a copy of your protected health information that may be used to make decisions about your care, including medical and billing records. To inspect and copy your protected health information, you must submit your request in writing to our Privacy Officer.

If you feel that the protected health information we have about you is incorrect or incomplete, you may ask us to amend the information. You have the right to request an amendment for as long as the information is kept by or for our practice.

You have the right to request a restriction or limitation on the protected health information we use or disclose about you for treatment, payment, or healthcare operations.

We reserve the right to change this notice. We reserve the right to make the revised or changed notice effective for protected health information we already have about you as well as any information we receive in the future. We will post a copy of the current notice in our office and on our website.

If you believe your privacy rights have been violated, you may file a complaint with our practice or with the Secretary of the Department of Health and Human Services. To file a complaint with our practice, contact our Privacy Officer. All complaints must be submitted in writing. You will not be penalized for filing a complaint.
